A Case of Adenoacanthoma of the Stomach


Abstract
A case of primary adenoacanthoma of the stomach is reported in view of its rarity and obscure origin.
The patient was a 38 years old male who has been complained of severe epigastric pain, melena, general weakness and weight loss since Feburary 1968.
On physical examination, he was emaciated, pale and irritable. Abdomen was moderately distended but no mass was palpablc. Upper G.I. series showed irregular polypoid mass at the fundus and cardiac portion of the stomach. Relative filling defect was demonstrated on fill up phase of X-ray film.
Total gastrectomy with splenectomy and partial pancreatectomy was followed by uneventful recovery. The excised specimen shown intact cardioesophageal junction and a tumor mass with ulceration which adherent to the tail of the pancreas and omentum. The hilus of the spleen was also invaded by tumor directly. Literatures regarding with squamous cell components of stomach carcinoma were reviewed.
Microscopically the tumor mass was composed of glandular cancer and squamous cell carinoma.
